POLIMORFISMOS DE LA METILENTETRAHIDROFOLATO REDUCTASA Y SU ASOCIACIÓN CON LOS DEFECTOS DEL TUBO NEURAL

Resumen

Los defectos del tubo neural (DTN) son un grupo de malformaciones
congénitas que comprenden la anencefalia, espina bífida y encefalocele; en
México, son las mas comunes de las malformaciones congénitas con una
incidencia de 3.6 por cada 1000 recién nacidos vivos y constituyen un
problema de salud pública (1). Los DTN se deben a una falla en el cierre del
tubo neural que ocurre en el primer mes del desarrollo embrionario que
ocasiona, en la mayoría de los casos, invalidez o muerte del infante en los
primeros días de vida (2). Aunque se desconoce el origen de estas
alteraciones, se ha descrito que las mujeres que consumen suplementos vitamínicos conteniendo
ácido fólico durante el período periconcepcional reducen las probabilidades de aparición de los
DTN (3). Se estima que los DTN son de origen multifactorial y se proponen alteraciones genéticas
y ambientales como factores de riesgo para su ocurrencia (4). En las primeras investigaciones se
observó que la mayoría de las mujeres con embarazos afectados con DTN presentan niveles
disminuidos de folato en plasma y en glóbulos rojos (5,6). También se encontró que las mujeres
con embarazos afectados e hijos con defectos del tubo neural presentan niveles plasmáticos
elevados de homocisteína y concentraciones disminuidas de folato y vitamina B-12 en sangre (7,8).
Como se describe posteriormente, en el metabolismo de la homocisteína participan enzimas que
utilizan al folato como cofactor y que resultan ser los candidatos ideales para ser considerados
factores de riesgo involucrados en los DTN prevenibles por folato. En este trabajo se analizan los
polimorfismos de la metilentetrahidrofolato reductasa (MTHFR), una de las enzimas que participa
en el metabolismo de la homocisteína y su asociación con los DTN.
